Specification is the hard part, not engineering
Almost every failed industrial software project we have been asked to rescue failed at specification rather than at build. The system modelled the industry rather than the business, and the difference showed up during testing when the cost of change was highest.
The reason is that the valuable knowledge in a manufacturing business is undocumented and frequently unconscious. The scheduler knows that a particular customer always revises the order after the first week, so nothing is cut until day eight. The storeman knows which supplier substitutes material grades without telling anyone. None of this appears in a requirements workshop, because nobody thinks to mention what they regard as obvious.
The only reliable way to find it is to watch the work. We do discovery on site over consecutive days rather than in scattered calls, and we insist on seeing a full cycle including month-end and at least one exception, because the exceptions are what the system will live or die on.

A production manager can take a call at ten in the morning between shifts, which is half past three our time and well inside our working day. A question about how a process actually works gets answered the same morning rather than becoming a blocker for a day. Making a manufacturer legible to a procurement buyer
The web work we do in the West Midlands is narrow and specific: making an engineering business evaluable by a buyer who is shortlisting before making contact.
The failure mode is universal in this sector. One process at a time, run in parallel with the existing method for a full cycle so nobody has to trust the new system before it has proved itself. Once it has, the old method stops, and we move to the next. Each step is short enough to abandon. This is slower than a single implementation and it is the reason our industrial projects finish.

5 hours a day where we are both at a desk
